Many women considering breast lift surgery, also known as mastopexy or a breast uplift, ask us the same question:

“I want my breasts lifted, but I don’t want implants unless they’re truly necessary.”

At Anka Plastic Surgery, breast implants are not considered an automatic part of breast lift surgery. In patients who have enough natural breast tissue, the breasts can often be lifted and reshaped using their own tissue alone.

A breast lift (mastopexy) is designed to correct sagging by removing excess skin, repositioning the nipple and reshaping the existing breast tissue. The aim is not necessarily to increase breast size, but to create a firmer, more balanced and proportionate breast shape.

Some patients, however, have significant volume loss, particularly after pregnancy, breastfeeding or major weight loss. In these cases, fat transfer or breast implants may be considered if additional fullness is needed.

The most appropriate treatment depends on your existing breast volume, degree of sagging, skin quality, anatomy and the breast shape you would like to achieve.

Who Is a Good Candidate for a Breast Lift?

A breast lift may be suitable if you:

  • Have breast sagging following pregnancy, breastfeeding, weight loss or ageing
  • Are satisfied with your breast volume but would like a firmer and more elevated shape
  • Have nipples that have descended towards or below the inframammary fold
  • Have stretched skin or downward-pointing nipples
  • Are in good general health and have realistic expectations

Patients who have both sagging and significant volume loss may benefit from combining mastopexy with fat transfer or breast implants.

Who May Need to Wait or Consider Alternatives?

The timing of surgery, or the choice of procedure, may need to be reconsidered in the following situations:

  • Currently smoking, because smoking increases the risk of wound-healing problems
  • Planning a pregnancy or breastfeeding in the near future
  • Significant weight fluctuation or plans for major weight loss
  • Medical conditions that are not well controlled
  • Significant volume loss, where fat transfer or an implant may also be needed
  • Expectations that cannot realistically be met by surgery

These points are always discussed individually during consultation.

Our Approach to Breast Lift Surgery

A successful breast lift involves more than removing excess skin. The breast tissue itself must also be reshaped and supported to create a balanced contour.

Whenever anatomy allows, we prefer to use the patient’s own breast tissue rather than routinely adding an implant. This implant-free breast lift approach may be particularly suitable for women who already have adequate breast volume but are mainly concerned about sagging or loss of shape.

We frequently use a medial pedicle technique in appropriate patients. This allows the nipple and areola to be repositioned while maintaining their blood supply and enables the existing breast tissue to be redistributed to improve breast shape.

The exact technique is selected individually according to the degree of breast sagging, tissue quality and the patient’s goals.

Breast Lift Techniques

The incision pattern used for mastopexy depends primarily on the amount of skin excess, degree of sagging and breast anatomy.

Breast lift incision patterns showing periareolar, vertical and inverted T techniques

Periareolar (Donut) Breast Lift

The incision is placed around the areola. This technique may be appropriate for selected patients with mild breast sagging.

Vertical (Lollipop) Breast Lift

The incision runs around the areola and vertically down towards the breast fold. It is commonly used for moderate degrees of sagging.

Inverted-T (Anchor) Breast Lift

This technique combines an incision around the areola, a vertical incision and an additional incision within the inframammary fold. It is generally used when there is more advanced sagging or significant excess skin.

The goal is not simply to choose the shortest possible scar. The incision pattern should allow the breast tissue and skin to be reshaped effectively enough to create a stable and balanced breast form.

Scars are permanent, although they normally soften, flatten and become less noticeable as they mature.

Do I Need Breast Implants with a Breast Lift?

No. Breast implants are not required for every mastopexy.

If you already have sufficient breast volume and your main concern is sagging, your existing breast tissue may be reshaped without adding an implant.

An implant may be useful when there has been substantial volume loss or when a patient specifically wants increased breast size or greater upper-pole fullness. For selected patients, fat transfer may provide a more modest increase in volume without using a breast implant.

The decision is therefore based on what is missing: position, volume, or both.

Preparing for Surgery

A detailed preparation plan is provided after your consultation. In general:

  • Online assessment: Your photographs, medical history, medications and the breast shape you would like to achieve are reviewed before you travel.
  • Medical assessment: Preoperative examination and required tests are completed in Istanbul before the procedure.
  • Breast imaging: Patients over a certain age, or with relevant family history, may be asked to provide recent breast imaging such as a mammogram or ultrasound.
  • Smoking and nicotine: Patients are advised to stop smoking and avoid nicotine products for the period recommended by the surgeon before and after surgery.
  • Medications and supplements: Blood-thinning medicines, anti-inflammatory drugs and some herbal supplements may need to be stopped beforehand. Never stop prescribed medication without medical advice.
  • Stable weight: A stable, healthy weight before surgery helps maintain the result.
  • Practical planning: Arrange help at home for the first days and loose, front-opening clothing for after surgery.

How Is Breast Lift Surgery Performed?

Breast lift surgery is performed under general anaesthesia.

Before surgery, detailed markings are made according to the planned nipple position and incision pattern. During the operation, excess skin is removed, the nipple and areola are repositioned where necessary, and the breast tissue is reshaped to create a more elevated contour.

If additional volume is required, fat transfer or a breast implant can be added during the same operation.

Most patients stay in hospital for one night and wear a supportive surgical bra during the early recovery period.

Recovery After Breast Lift Surgery

Recovery varies according to the extent of surgery and whether mastopexy is performed alone or combined with another procedure.

First Week

Mild to moderate discomfort, swelling and tightness are common during the first few days and can usually be controlled with prescribed medication.

Patients are encouraged to walk soon after surgery and should wear their surgical bra as instructed. Many patients with desk-based work feel comfortable returning after approximately one week.

Weeks 2–6

Swelling gradually decreases and normal daily activity can be increased progressively. Heavy lifting, strenuous upper-body activity and intensive exercise are usually restricted for approximately 4–6 weeks, depending on healing and your surgeon’s assessment.

Following Months

The breasts gradually settle into a more natural position. Scars also continue to mature, soften and fade over several months.

Scar Care

Once the wounds have healed and your surgeon has approved it, silicone gel or silicone sheets may be recommended to support scar maturation. Protecting the scars from direct sun exposure is important during the first year. Scars cannot be removed entirely, but they usually become paler and flatter over time.

Risks and Safety

As with any surgical procedure, breast lift surgery carries potential risks. These may include:

  • Bleeding or haematoma (collection of blood)
  • Infection
  • Delayed wound healing
  • Changes in nipple or breast sensation
  • Reduced blood supply to the nipple-areola complex, which in rare cases may affect part or all of the nipple
  • Asymmetry
  • Unfavourable or widened scarring
  • Anaesthetic risks and, rarely, blood clots
  • The possibility of revision surgery

Smoking can significantly increase the risk of wound-healing problems and should be discussed during your consultation.

Your individual risks, expected outcome and alternative treatment options will be explained before surgery so that you can make an informed decision.

Planning Your Surgery from Abroad

International patients can begin the assessment process remotely.

For an initial evaluation, we usually request:

  • Clear front and side photographs
  • Age, height and weight
  • Information about previous breast surgery
  • Relevant medical conditions and medications
  • A description of the breast shape and size you would like to achieve

Assoc. Prof. Anı Çinpolat reviews this information and recommends the most appropriate surgical approach.

Most international patients remain in Istanbul for approximately one week after surgery so that early postoperative checks can be completed before travelling home.

Frequently Asked Questions

Will I have visible scars?

Yes. All breast lift techniques require incisions and therefore leave scars. Their position depends on the surgical technique used. Scars usually become softer and less noticeable as they mature.

How long is the recovery after a breast lift?

Many patients return to light daily activities within approximately one week. More strenuous exercise and heavy lifting are generally postponed for around four to six weeks.

Will my breasts look natural after a breast lift?

The aim of mastopexy is to create a breast shape that is balanced with your anatomy. The achievable result depends on your existing breast tissue, skin quality and starting breast shape.

Can an implant-free breast lift give good results?

Yes. In patients with adequate natural breast volume, the existing breast tissue can often be reshaped without using an implant.

What is the difference between a breast lift, breast augmentation and breast reduction?

A breast lift corrects position and sagging, usually without changing size much. Breast augmentation increases volume. Breast reduction decreases volume and also lifts the breast. Some patients need a combination, for example a lift with an implant or fat transfer. This is decided at consultation. See our Breast Augmentation and Breast Reduction pages for more detail.

Will nipple sensation change after a breast lift?

Temporary changes in nipple or breast sensation are common after surgery and often improve over time. Permanent changes are less common but possible. This is discussed in detail during consultation.

Can I become pregnant after a breast lift?

Yes. However, pregnancy and breastfeeding can alter breast volume and skin elasticity and may therefore change the result of a previous breast lift.

Will I be able to breastfeed after surgery?

Breastfeeding ability can often be preserved, but it cannot be guaranteed. The effect depends on the surgical technique and individual anatomy.

How long do breast lift results last?

The improvement is long lasting, but breast shape will continue to change naturally with ageing, gravity, pregnancy and significant weight fluctuations.

When can I travel home after surgery?

Most international patients stay in Istanbul for approximately one week. The exact timing of travel is determined according to postoperative recovery and examination findings.
