GLP-1 Analogues (Ozempic, Mounjaro) and Facial Sagging: What Is “Ozempic Face”?

Recently, an increasing number of patients using GLP-1 analogues such as Ozempic and Mounjaro for weight loss have been presenting to our clinic with concerns like
“my face looks saggy” or “my face has collapsed.”

The primary reason for this change is not the medication itself, but rather rapid and uncontrolled fat loss.

Why does the face get affected?

During rapid weight loss:
• Facial fat compartments diminish significantly
• The skin cannot adapt quickly enough to the sudden volume loss
• Structural support of the face weakens

As a result, the face may appear:
• more saggy
• more tired
• noticeably older

This condition is commonly referred to as “Ozempic face.”

How do we approach treatment?

Our main goal in these cases is to restore the structural support that the face has lost, rather than simply adding volume.

Areas most commonly requiring support include:
• cheekbones
• midface
• under-eye region
• jawline

Depending on the patient, we use dermal fillers or fat grafting to re-establish facial balance.

The critical point is:

The goal is not to overfill the face, but to restore lost anatomy.

Excessive filler can:
• weigh the face down
• distort natural facial expressions
• lead to an unnatural appearance

Therefore, a minimal, balanced, and anatomy-based approach is essential.

Why is additional support important after the age of 40?

In patients over 40, the issue is not only volume loss; skin quality also plays a significant role.

For this reason, filler treatments are often combined with:
• mesotherapy
• PRP
• exosome treatments
• energy-based skin tightening methods such as HIFU

These combinations help achieve more natural and longer-lasting results.

Can facial sagging be prevented in patients planning to use these medications?

Yes, it can largely be prevented or at least significantly slowed down.

Key factors include:
• adequate protein intake
• small and frequent meals
• avoiding excessively rapid weight loss

In addition, before sagging becomes noticeable, preventive low-dose filler support to areas prone to volume loss can help the face adapt more smoothly to the weight-loss process.

If you notice significant changes in your face while losing weight, addressing the issue early can lead to much more satisfying and natural-looking results.
